Patient Navigation Services (PNS)
CaseSherpa also provides patient navigation services which are often used in addition to a traditional CCM offering. While similar to CCM services, patient navigation services are typically utilized to achieve specific therapeutic or behavioral goals for patients. Examples of patient navigation support provided by CaseSherpa include:
- Surgical Navigation
- Diagnostic Testing
- Social Determinants
- Pharmacy Validation
- Specialists Coordination
These services are typically NOT directly reimbursed by insurance providers, but are used to generate additional revenue opportunities, reduce unnecessary costs, or provide expanded support for patients in need
