CaseSherpa Ventures, LLC (“CaseSherpa,” “we,” “us,” or “our”) is committed to protecting the privacy of our users and patients. This Privacy Policy describes how we collect, use, and safeguard your information.

Information We Collect

We collect information provided by your healthcare provider as part of your clinical care enrollment, including your name, phone number, email address, and health-related information necessary for care coordination.

How We Use Your Information

We use your information to:

    • Coordinate and manage your clinical care programs

    • Send appointment reminders and confirmations

    • Deliver telehealth scheduling links and meeting invitations

    • Send health screening surveys

    • Communicate care coordination and follow-up messages

    • Send post-procedure check-in communications

HIPAA Compliance

CaseSherpa is a HIPAA-compliant platform. All patient health information is stored in encrypted databases and handled in accordance with HIPAA Privacy and Security Rules. We maintain Business Associate Agreements (BAAs) with all third-party service providers that handle protected health information.

SMS / Text Messaging Terms & Consent

By providing your mobile phone number to CaseSherpa or your healthcare provider during clinical enrollment, you expressly consent to receive automated text messages (SMS/MMS) from CaseSherpa Ventures, LLC related to your healthcare.

Messages you may receive include:

    • Appointment reminders and confirmations

    • Telehealth scheduling links and meeting invitations

    • Health screening and bowel control survey links

    • Care coordination and follow-up messages

    • Post-procedure check-in communications

    • Enrollment and consent-related notifications

How You Opt In: Your healthcare provider enters your phone number into the CaseSherpa platform as part of your clinical care enrollment. By providing your number and proceeding with enrollment, you consent to receive the text messages described above. You may also opt in at any time by texting START to +1 (601) 821-2724.

Message Frequency: Message frequency varies depending on your care program, appointment schedule, and treatment stage. You may receive approximately 2-5 messages per month.

Message & Data Rates: Standard message and data rates may apply depending on your mobile carrier and plan. CaseSherpa does not charge any additional fees for text messages.

How to Opt Out: You may opt out at any time by replying STOP to any message from CaseSherpa. You will receive a one-time confirmation message, and no further messages will be sent. You may also request removal by emailing admin@casesherpa.com.

SMS Consent Is Not Required: Consent to receive text messages is not a condition of purchasing any goods or services. If you do not wish to receive text messages, please inform your healthcare provider.

SMS Privacy: Phone numbers collected for SMS communication are never sold, rented, or shared with third parties for marketing purposes. Phone numbers and SMS message history are stored in encrypted databases in compliance with HIPAA security requirements. Our SMS service provider is Twilio, Inc., which maintains a signed HIPAA Business Associate Agreement (BAA) with CaseSherpa.

Supported Carriers: Major carriers supported include AT&T, Verizon, T-Mobile, Sprint, and most regional carriers.

Sample Consent Form & Verbal Script

The following is a sample of the consent language used by CaseSherpa partner clinics at the point of clinical enrollment. Patients provide consent in two
ways before any SMS messages are sent: (1) verbally to clinic staff, and (2) by signing a written authorization form during patient intake.

Sample Verbal Consent Script (read by clinic staff at intake)

“As part of your enrollment in our CaseSherpa care management program, we would like to send you text message notifications related to your care — including appointment reminders, telehealth visit links, screening surveys, and care coordination updates. Message frequency varies, and message and data rates may apply. You can stop these messages at any time by replying STOP, or get help by replying HELP. Consent is not required to receive care. Do you consent to receive these text messages from CaseSherpa?”

Sample Written Consent (excerpt from clinical intake / HIPAA authorization form)

“By signing below and providing my mobile phone number, I authorize CaseSherpa Ventures, LLC to send me automated text messages (SMS/MMS) related to my healthcare, including appointment reminders, telehealth links, screening surveys, and care coordination communications.

I understand that:
• Message frequency varies
• Message and data rates may apply
• I may opt out at any time by replying STOP to any message
• I may request help at any time by replying HELP
• Consent to receive text messages is NOT required as a condition of
receiving healthcare services
• My mobile number and SMS content will never be sold, rented, or shared
with third parties for marketing purposes
• CaseSherpa’s SMS service provider, Twilio Inc., maintains a signed
HIPAA Business Associate Agreement with CaseSherpa”

How Patient Numbers Are Entered

Once consent is documented, clinic staff enter the patient’s mobile number into the CaseSherpa platform as part of clinical enrollment. The patient then receives an initial SMS related to their care program. Patients may also opt in by texting START to +1 (601) 821-2724.

 

 

Third-Party Service Providers

We use the following HIPAA-compliant service providers to deliver our platform:

    • Amazon Web Services (AWS) — cloud hosting, email delivery, database

    • Twilio — SMS messaging

    • Zoom — telehealth video visits

Each provider maintains a signed Business Associate Agreement (BAA) with CaseSherpa.

Data Security

We implement industry-standard security measures to protect your information, including encryption at rest and in transit, access controls, and audit logging.
